Low-molecular-weight heparin (LMWH) has been considered theoretically superior to unfractionated heparin in several facts such as more effective anticoagulant, more predictable bioavailability, and less bleeding complications. In rare cases, LMWH is associated with spontaneous major bleeding complications.
We report a case of LMWH, enoxaparin-induced spontaneous intramuscular hematoma with compartment syndrome in the left lower leg in a 79-year-old woman with atrial fibrillation. We used enoxaparin (1 mg/kg per 12 hours) as bridging anticoagulation before use of warfarin in patient with atrial fibrillation. At 3 days of used for enoxaparin, large and painful subcutaneous mass was noticed in the mid portion of left thigh in condition that had not intrmuscular injection or trauma. It was associated with pulseless in the left dorsalis pedis artery and pale and tingling sense in the left lower leg. We diagnosed large intramuscular hematoma by computer tomography and immediately and successfully treated patient with surgical removal of hematoma.(Korean J Med 69:S948-S951, 2005)
